We maintain HIPAA-compliant electronic versions of your healthcare provider’s forms. Fill one out on the device in your pocket. We email a PDF to their office. No more fumbling with clipboards! 🤠
Technology is an equalizer that eliminates struggles with pen & paper. Many people have problems with writing because of limited dexterity, limited vision, or other health conditions. Our forms comply with Section 508 and WCAG 2.1 AA standards.
Our forms collect lots of electronic protected health information (ePHI). While AlpineForm is designed for simplicity, we adhere to strict HIPAA and HITECH compliance standards to keep your ePHI safe & secure. All collected ePHI is for your provider’s use only.
Healthcare providers use a lot of paper. Paper comes from trees. Trees make forests. Electronic forms don’t require paper. Eliminate paper, save some trees, and be friendly to forests. It’s a little way to help preserve our great outdoors.
We’re Scott & Sarah Pruett, a husband and wife team. Scott has quadriplegia from an alpine skiing accident, and Sarah is a registered and licensed Occupational Therapist with years of experience treating patients under HIPAA regulations.
Scott has no fine motor control in his hands due to neurological damage from his spinal cord injury. We created AlpineForm out of frustration with him needing assistance to fill out paper forms.
We also co-founded a business called The UD Project . A portion of AlpineForm’s profits goes toward advancing universally accessible housing.
