Our forms collect lots of electronic protected health information (ePHI). While AlpineForm is designed for simplicity, we adhere to strict HIPAA and HITECH compliance standards to keep your ePHI safe & secure. All collected ePHI is for your provider’s use only.

Friendly to Forests.

Healthcare providers use a lot of paper. Paper comes from trees. Trees make forests. Electronic forms don’t require paper. Eliminate paper, save some trees, and be friendly to forests. It’s a little way to help preserve our great outdoors.

About Us.

We’re Scott & Sarah Pruett, a husband and wife team. Scott has quadriplegia from an alpine skiing accident, and Sarah is a registered and licensed Occupational Therapist with years of experience treating patients under HIPAA regulations.

Scott has no fine motor control in his hands due to neurological damage from his spinal cord injury. We created AlpineForm out of frustration with him needing assistance to fill out paper forms.


We also co-founded a nonprofit called The Universal Design Project . A portion of AlpineForm’s profits goes toward advancing universally accessible housing.
